Chapter 4 – Addressing the Social Determinants of Health: ACCHO practices and principles

 

This chapter describes ACCHO ways of working to address the social determinants of health impacting their clients and communities. ACCHOs act as a one-stop-shop for community through internal holistic services and programs and through partnerships with external service providers. ACCHO staff work with clients to identify their needs before doing whatever is necessary to support them.

The content within this chapter was drawn from an in-depth case study with a Regional ACCHO that was reviewed and refined by the CREATE Leadership Group and strengthened with learnings and perspectives from the ACCHO sector nationwide.
